If you had to guess which single measurement best predicts how long someone will live, you’d probably guess blood pressure, cholesterol, or maybe VO2 max. Most people would not guess grip strength. And yet, in some of the largest studies ever done on this question, a simple hand-dynamometer squeeze outperforms several of the metrics we obsess over in annual physicals.
The Study That Changed the Conversation
In 2015, The Lancet published findings from the Prospective Urban Rural Epidemiology (PURE) study, which measured grip strength in more than 140,000 adults across 17 countries. The results: every 5-kilogram decrease in grip strength was associated with a 17% increase in cardiovascular mortality, along with higher rates of death from any cause. When the researchers compared grip strength directly against systolic blood pressure as a predictor of cardiovascular death and all-cause mortality, grip strength won. A hand squeeze, tracked at scale, told them more than the number their doctor writes down at every appointment.
This isn’t a one-off finding. It’s been replicated across multiple large cohorts in the decade since, which is part of why grip strength has become a standard inclusion in geriatric and longevity research.
Why Would Hand Strength Matter That Much?
It sounds like an odd thing to matter so much, until you understand what grip strength is actually measuring.
First, grip strength is a reliable proxy for overall muscle mass and neuromuscular function. It’s not really “about” your hands — it’s a readout of how well your whole musculoskeletal and nervous system is aging, because the muscles, tendons, and nerves that produce a strong grip are wired into the same systems that determine whether you can carry groceries, catch yourself in a fall, or get up off the floor unassisted. Grip strength declines in step with the broader process of sarcopenia (age-related muscle loss) — so measuring your grip is, in effect, a cheap and fast way to measure something much bigger.
Second, gripping something hard — really holding on — recruits a coordinated effort from your entire upper kinetic chain: forearm, shoulder, upper back, and core all engage to stabilize the effort. A weak link anywhere in that chain shows up as a weaker grip. It’s a compound signal, not an isolated one.
Since it measures how your nervous system is firing, we can also use it as a recovery gauge in serious athletes. Tracking grip strength from week to week and establishing a baseline allows us to see when you’re below baseline (i.e. not recovered) and when you’re primed (well above baseline).

Where You Should Stand
Normative data gives a useful reference point. For men aged 25–39, healthy grip strength on the dominant hand typically falls in the 46–56 kg range. For women in the same age bracket, 25–30 kg is the healthy range. If your numbers are meaningfully below these — and especially if they’re trending downward year over year — that’s a signal that it’s time to start training your grip strength. A decline is not inevitable.
The Good News: It’s Trainable
Grip strength doesn’t require a specialized program to improve. Most compound strength movements already train it as a byproduct:
- Deadlifts — arguably the single best grip-strength exercise that exists, since heavier loads force your hands to hold on longer under real tension.
- Rows — any pulling movement (barbell, dumbbell, cable) doubles as grip work.
- Pull-ups and hangs — direct, unavoidable grip demand.
- Farmer’s carries — walking for distance or time while holding a heavy weight in each hand is one of the most underrated grip-and-core exercises available.
If you want to accelerate progress beyond what those movements deliver on their own, dedicated grip work helps: dead hangs from a pull-up bar, thick-bar training (fat grips, kettlebells, sandbags), or a simple grip trainer used for a few minutes several times a week.
Test Yourself
You don’t need a hand dynamometer to get a useful read. Here’s a simple field test: find a pull-up bar, take a full grip, and hang with your feet off the floor. Time how long you can hold on before your grip fails.
Scoring:
- Under 30 seconds — grip work should become a deliberate part of your training
- 30 seconds — a reasonable minimum baseline
- 60 seconds — strong
Test it periodically — monthly is plenty — and treat a declining number the same way you’d treat a rising resting heart rate: a signal worth investigating, not ignoring.
The Bottom Line
Grip strength isn’t really about your hands, and it isn’t a vanity metric. It’s one of the clearest, cheapest, most heavily-researched windows we have into how your whole body is aging — validated at a scale (140,000 people, 17 countries) that few fitness metrics can claim. The fix isn’t complicated: keep pulling, keep carrying heavy things, and keep hanging on.
