The Balance Test That Predicts How You’ll Age

Try this balance test before you read any further: stand on one leg, close your eyes, and see how long you can hold it.

Most people last a few wobbly seconds. Some can’t get their foot off the floor before they’re catching themselves on a wall. It feels like a party trick. It isn’t. It’s one of the more quietly predictive tests in all of fitness science, and most people have never been asked to do it.

The Study Behind the Test

A 2022 paper published in the British Journal of Sports Medicine tracked middle-aged and older adults (roughly 51–75 years old) and had them attempt a 10-second single-leg stance with their eyes closed. The people who couldn’t complete it had an 84% higher risk of death from any cause over the following decade, compared to those who could. Eighty-four percent. From a balance test.

That number holds up even after adjusting for age, sex, and existing health conditions. Balance isn’t a proxy for something else going on in these studies — it’s an independent signal, and a strong one, about how your whole system is aging.

Why Balance Matters So Much

Balance feels simple, but it’s the output of several systems working together in real time:

  • Strength — particularly in the ankles, hips, and core, which make the small, constant corrections that keep you upright.
  • Proprioception — your body’s sense of where your limbs are in space without looking at them.
  • Vestibular function — the inner-ear system that tracks head position and motion.
  • Neuromuscular coordination — the speed and precision with which your brain and muscles communicate to correct a wobble before it becomes a fall.

All four of these degrade with age if they’re not deliberately challenged — and unlike strength loss, which you can usually feel happening, balance erodes quietly. Most people don’t register how much they’ve lost until a near-fall (or an actual one) forces the issue.

The Real Stakes: Falls

This isn’t an abstract fitness metric. Falls are among the leading causes of injury-related death in adults over 65 in the United States — a bigger contributor than most people assume, and one that gets far less attention than it deserves. The danger isn’t just the fall itself. It’s the cascade that follows: reduced mobility during recovery, loss of independence, faster muscle loss from time spent inactive, and in many cases an accelerating decline that a person never fully recovers from. A hip fracture in someone over 65, for instance, carries a well-documented 20–30% one-year mortality rate — a statistic about survival, not just quality of life.

Balance training is one of the few interventions that directly targets the mechanism behind that cascade, before it starts.

Balance Is Trainable — And It’s Already in Good Programming

The reassuring part: balance responds to training the same way strength does. You don’t need special equipment or a dedicated “balance day.” Single-leg exercises — lunges, step-ups, single-leg deadlifts, Bulgarian split squats — challenge exactly the systems described above, and most well-designed strength programs already include them. The difference between a program that protects your balance and one that doesn’t is often just intentionality: are you doing these movements deliberately, tracking difficulty and range of motion, or are you skipping straight to the exercises that feel more “productive”?

Beyond standard programming, a few things accelerate the process:

  • Closed-eye variations — removing visual feedback (as in the test itself) forces your proprioceptive and vestibular systems to do more of the work, which is exactly the adaptation you’re after.
  • Reactive drills — small, controlled perturbations (a coach nudging you off balance, catching a tossed ball while on one leg) train the speed of your correction, not just your static hold time.

A few dedicated minutes, two to three times a week, is enough to move the needle. This is not a large time investment relative to what it protects.

Test Yourself

Here’s the protocol, in full: stand on one leg, close your eyes, and time how long you can hold the position without your raised foot touching down or your standing foot shifting to catch yourself. Use a wall or countertop nearby for safety on your first attempt.

Scoring:

  • Under 10 seconds — a genuine red flag worth addressing soon
  • 10–30 seconds — average, with real room to improve
  • 30–60 seconds — good
  • 60 seconds — excellent

Try it on both legs. Most people find one side is noticeably weaker, which is itself useful information.

The Bottom Line

Balance is one of the least glamorous things you can train and one of the most predictive of how your last few decades will actually go. It doesn’t show up on the scale, it doesn’t make for an impressive Instagram post, and it’s easy to skip in favor of the parts of training that feel harder. But the data is hard to argue with: the ability to stand on one leg for 10 seconds in middle age is telling you something real about your future.
